What's with dreaming?

Everyone dreams. But most people soon forget their dreams after waking up, leaving only some feelings at most. Those who still remember the dream clearly after waking up can only sleep badly or dream more.

Why do insomniacs always say MUBI?

It is often said that "I didn't have a good rest last night and had a dream all night" and "I was in MUBI at night and didn't fall asleep at all". So it seems that I really feel a little depressed and lack of energy when I study and work on this day, as if dreaming really delayed my rest. According to the traditional concept, some doctors also admit that dreaminess is a sign that the brain has never rested or has a bad rest. In fact, this old concept is wrong. Dreaming and recalling dreams are not indicators of poor sleep, nor can we say that dreaming means poor sleep. As mentioned earlier, dreams are a common physiological phenomenon. Dream or not, have 4 to 5 dreams every night. It is an exaggeration to say that you dream all night, and it is unrealistic to say that you don't dream, not to mention that MUBI dreams less. Some people at home and abroad have made physiological tests on insomnia, dreaming all night or MUBI's chronic insomnia, and carried out EEG tracing and research with various physiological instruments. It is proved by objective records that these patients who complain of "MUBI" and "dreaming all night" have the same sleep cycle as normal people, and the proportion and actual time of their fast-wave sleep period with dreams have not been significantly shortened or prolonged. "Dreaming all night" and "MUBI" have the same experience. Others give psychological tests to such patients, believing that this experience has something to do with personality. No matter from the universal law of human sleep or from the results of experimental research, dream feeling and insomnia may not be necessarily related. Whether or not you have a dream feeling and the degree of it can't be used as an objective indicator of insomnia.

In that case, why do people always associate dreaming with insomnia? As we have said many times before, dreams are inevitable physiological phenomena and active physiological processes during fast wave sleep, which are manifested in the forms of performance components, perceptual memory activities and unconventional associations. Dream feeling refers to the memory of some plots in the dream after waking up, or just the impression of having a dream, and even the content of the dream is difficult to explain clearly. People who complain about insomnia and dreamless dreams should refer to the feeling of dreams, which is related to emotional factors and personality characteristics. For example, introverts pay more attention to their inner feelings, sleep lightly and are easy to wake up or wake up, and often recall vivid dreams; People who are depressed and anxious are easy to wake up from their dreams, so they feel dreamy and sleep uneasily. The researchers also found that MUBI is closely related to the degree of waking up during sleep, and people who report themselves in MUBI on weekdays are more likely to wake up during sleep. In the sleep experiment, as long as the volume of 20 decibels is enough to wake them up, and their dream memory is also very high. Clinically, people who often see sudden dreaminess are often related to being hurt by seven emotions, being in a bad mood and sleeping uneasily, so insomnia is often accompanied by dreaminess.

Clinical observation and experimental results prove that the chief complaint of insomnia, especially long-term insomnia, is unreliable. Many scholars have studied the patients who complained of insomnia and found that insomnia is related to psychological disorders. They often overestimate the sleep latency and underestimate the actual sleep time. During the experimental test, the description of sleep parameters by insomnia patients is inconsistent with the objective records, either exaggerated or narrowed. Most experts believe that the experience of insomnia is related to mood and personality.

Why do many insomniacs complain about "dreaming all night or MUBI"? Some people think it is related to the following factors through investigation and analysis:

(1) Little is known about the knowledge of sleep and the relationship between sleep and dreaminess: many people don't know the alternation of sleep cycles, the position of dreams in sleep, physiological functions and the difference between dreams and dreaminess, and they don't know that dreams or dreaminess have no side effects. The unscientific interpretation of dreams by old traditional concepts and feudal superstitions has caused people to be afraid of dreams, which has led many people to blindly fall into fear of dreams.

(2) It is related to people's emotional state: many people lack understanding of emotional disorder, don't know that emotional disorder is a disease, and often ignore emotional disorder itself and pay too much attention to insomnia, dreaminess, pain and other symptoms related to emotional disorder. I don't know how to adjust and improve my mood, but I just exaggerate my less objective experience.

(3) paying too much attention to one's own health and dreams leads to the enhancement of dreams; On the other hand, the increased sense of dreams aggravates the fear of health and insomnia, and even forms a vicious circle.

(4) Some people wake up in the fast wave sleep period or wake up immediately after the fast wave sleep period, which leads to the increase of dream memory.

(5) Differences in individual functional states: Different individuals have different dreams, even if the same individual has different functional states in different periods, the degree of dreams is not the same. Therefore, some people have a strong sense of dreams for a while (MUBI), while others have a weak sense of dreams (fewer dreams).

Insomnia as a phenomenon exists objectively, and dreams as a common physiological phenomenon are by no means redundant. We admit the existence of insomnia, but we can't think that insomnia and dreaming are necessarily related.

How does Chinese medicine recognize the relationship between dreaminess and insomnia?

Traditional Chinese medicine believes that the root cause of dreaminess is the internal changes of the body. "Fang Suwen's Theory of Rise and Fall" said: "It is fascinating to be dumbfounded because of lack of gas." The so-called qi deficiency means that the qi is insufficient, and the yang fails to keep the yin and the god is unprepared, so it is a dream. This is also one of the reasons. However, emotional damage, visceral injury, loss of essence, make you uneasy and dreamy; Yin and blood deficiency can not nourish the mind, hide the liver and soul, limit the fire, and make the mind wander and dream more; Phlegm heat interferes with the liver and gallbladder, so the soul is restless and dreamy; Overwork, fire and water are not good, heart and kidney are not harmonious, people are uneasy and dream more; A dishonest diet makes the soil dull, restless and dreamy. The appearance of dreaminess leads to restless sleep and changeable dreams.

Can emotional stimulation also cause dreaminess? How to correct it?

As we have introduced in the relevant items, dreaming is essential for every normal person, and there are some benefits. So why do many people worry about dreaming? They think that dreaming affects the sleep effect and causes many uncomfortable symptoms during the day. Actually, it's not. If you ask these people carefully, you will find that they have all kinds of psychological factors. It is these psychological factors that affect the normal sleep at night and also cause other symptoms. They can also experience the pain caused by troubles through dreams. The content of some people's dreams has something to do with the reasons why they are unhappy. However, due to psychological factors, the increased sleep time is occupied by "dreamless sleep", but "dreamless sleep" has not increased, so the sleep depth has become shallow. This kind of sleep can't make the brain get enough rest and recovery, but it increases the stimulation. Objectively speaking, sleep is prolonged, but the effect is worse. When you wake up, you will feel tired and uneasy. Every time I wake up from a nightmare at night, I will subjectively think that I had a dream all night.

"Dreaming all night" to relieve symptoms; To improve the sleep effect, you can take some sleeping pills to reduce "dreamless sleep" and increase the depth of "dreamless sleep". However, this can only be used as a temporary symptomatic treatment, not a palliative. If you take drugs for a long time to deprive you of "dream sleep", you will also have "dream pressure". Once the drug is stopped, there will be a "rebound phenomenon". In this way, the content and time of dreaming will be significantly increased than before taking medicine, so that patients dare not stop taking medicine, fearing that stopping medicine will bring more pain. If you don't stop taking the medicine, you will be dependent on the medicine for a long time. Therefore, taking sleeping pills can only be used as an adjuvant treatment.

The fundamental method of treatment is to remove the psychological factors that lead to poor sleep. If patients face psychological difficulties through psychotherapy and specific guidance, adopt correct methods and attitudes, and gradually solve practical difficulties and contradictions, painful dreams will be reduced, sleep will return to normal, and many other symptoms will disappear. In short, the psychological factors of trouble are gone, and a series of reactions caused by it disappear.

Night grinding is a phenomenon, not a disease, just like dreaming and talking in your sleep. Excessive emotional tension or excitement, bad occlusion habits, intestinal parasite infection, etc. , often increase the number of night molars. Severe bruxism will accelerate tooth wear, lead to symptoms of tooth allergy, and even cause periodontal tissue damage, masticatory muscle fatigue and temporomandibular joint dysfunction.

The prevention and treatment of nocturnal molars should start from the etiology in order to get good results.

Eliminate tension

Get rid of unnecessary worries, be open-minded, arrange your work reasonably, and take diazepam orally if necessary, 1-2 times, one tablet each time.

(2) Develop good living habits

Regular daily life, don't eat too much dinner, don't do strenuous exercise before going to bed, especially children should develop the habit of hygiene.

(3) Suspected intestinal parasites

Carry out anthelmintic treatment under the guidance of a doctor to reduce the peristalsis of intestinal parasites and stimulate the intestinal wall.

(4) Correct the bad habits of oral system.

For example, chewing on one side, biting a pencil, wrong? Wait a minute.

(5) The above treatment has not improved.

Should I go to the stomatological hospital for dentition? Pad, put it on the dentition before going to bed at night and take it off in the morning. ? Can pad be eliminated? Interference, relieve muscle tension.

After nightfall, most people have fallen asleep. However, some people (especially children) grind their teeth when they sleep. Because there is no food in the mouth when grinding teeth at night, saliva secretion is correspondingly reduced, and teeth are not lubricated, thus forming a "dry molar" similar to a hollow molar. If you often "dry your teeth", the wear on your teeth will be quite serious. After a long time, the worn teeth will have different degrees of acidity or pain, and sometimes they will form mandibular joint disorder.

Why do people grind their teeth at night?

It turns out that the action of grinding teeth is caused by the continuous contraction of masticatory muscles. Masticatory muscles are managed by the motor branch of the third branch of trigeminal nerve. As for why the exercise club is so excited, why the human body is ordered to contract the masticatory muscles without eating or drinking at night, it is not particularly clear. But from the clinical point of view, we can often see cases of ascariasis and gastrointestinal dysfunction leading to molars.

Ascaris lumbricoides parasitized in human intestines can secrete many toxins, such as neurotoxins, allergic toxins, hemolytic toxins and enzymatic toxins. In addition, some metabolites excreted by worms will constantly stimulate the corresponding parts of the sleeping human brain, so that some people's masticatory muscles will continue to contract at night, leading to molars. Studies have found that if children are too excited during the day, or too tired during the day, and too busy and tired before going to bed at night, although most of the cerebral cortex is in a state of inhibition, some areas (such as trigeminal nerve of pons) will be in an excited state after falling asleep. This will send a "signal" to command the facial masticatory muscles to contract for a short time. Therefore, people who sleep will show that their jaws will involuntarily move up and down, left and right, and then make a "squeaky" grinding sound. The sound of grinding teeth at night is clearly heard by others, but the people who grind their teeth are completely unaware of it.

How to grind your teeth at night?

If the patient has ascariasis, it should be thoroughly deworming. If it is purely caused by mental and psychological factors, adults should combine work and rest, while children should pay attention not to be too excited and tired during the day. If you grind your teeth badly at night, you can take a sedative before going to bed under the guidance of a doctor. For patients with gastrointestinal dysfunction, medication should be taken in time to regulate gastrointestinal function. In short, as long as symptomatic treatment, night grinding can receive ideal therapeutic effect.

Bruxism is a habit of grinding teeth habitually when sleeping or unconsciously when awake. The etiology of night molars is still unclear, and it is considered to be related to many factors such as spirituality, emotionality, odontogenesis, systematicness, professionalism and spontaneity.

1. Mental factors The oral cavity has the function of expressing tension. Patients with fear, anger, hostility, conflict and other emotions, especially anxious patients, try to relieve their inner depression by grinding their teeth for some reason. These patients have frequent contact with teeth for a long time. These psychological factors may be one of the important causes of bruxism. 2. Occlusal factors In neurotic individuals, any occlusion interference may be the trigger of bruxism; The occlusal factors of bruxism patients are mostly the mismatch between the median relationship and the median occlusion, and the premature contact of the non-working side during lateral occlusion; Moreover, the clinical occlusal adjustment method has successfully cured some bruxism. Occlusal factor is an important factor of oral health, but whether it causes bruxism is still controversial. When changing teeth, the jaw-biting relationship is disordered, which leads to the movement disorder and spasm and contraction of the progenitor muscles; 3. Central Nervous Mechanism At present, there is a tendency to think that grinding teeth, like sleepwalking, enuresis and nightmares, is an involuntary subconscious action, a symptom of partial awakening of the brain during sleep, a central sleep disorder related to daytime emotions, and is caused by internal or external, psychological or physiological sleep interference stimulation. 4. Other systemic factors, such as gastrointestinal dysfunction related to parasites, nutritional deficiency in children, blood sugar and calcium concentration, endocrine disorders, allergic reactions, etc., may become the pathogenic factors of bruxism. Some cases have genetic factors. In addition, hyperuricemia, hyperthyroidism, allergy and bladder stress may all be related to bruxism. 5. Professional factors, such as drivers and athletes, and jobs that require high precision, such as clockmakers, are easy to be worn out.

The most serious harm is to grit your teeth and grind your teeth at night. Nocturnal molars are more common in children. Although there is no pain in grinding teeth at night for the time being, it will cause serious wear and tear on the occlusal surface and adjacent surface of teeth after a long time, and all kinds of diseases mentioned above will be very complicated. Refractory bruxism can lead to periodontal tissue destruction, tooth loosening or displacement, gingival recession and alveolar bone loss. During the day, when we chew food, we also rub our teeth, but the damage to our teeth is very small. This is because when we chew, the food between our upper and lower teeth is like a cushion, and there is enough saliva to make our teeth smooth, so it is not easy to wear. It seems that I chewed for a long time while eating, but I moved up and down most of the time. Some people have studied that the real direct contact between teeth is only about 4 minutes. If you grind your teeth at night, the situation will be very different. There is no food in the mouth, and the secretion in the mouth is also reduced. Your teeth don't get the necessary lubrication, so they form a "dry grinding". It's really like the mill is empty. This kind of tooth wear is very serious, and the consequences are quite serious. At this time, the worn teeth often have different procedures of acidity or pain, and sometimes cause temporomandibular joint dysfunction, destroying the shape and function of the occlusal system. It can also cause masticatory muscle dysfunction, such as masticatory muscle hyperactivity, spasm, fatigue and pain. Unbalanced muscle contraction, abnormal occlusion, change of vertical height between jaws and imbalance of disc process position also directly affect the normal shape and function of temporomandibular joint, which is manifested as pain at the joint, joint snapping and limited mouth opening. Pain is depressing and depressing, especially when you get up in the morning. People who grind their teeth at night will affect others' sleep.

Prevention and control; Don't do it when it's lighter. Pay attention to rest. Psychotherapy can remove pathogenic factors, adjust occlusion and treat systemic diseases related to bruxism. Avoid excessive excitement during the day. Relax before going to bed and don't watch exciting TV movies. Go to the hospital to check intestinal parasitic diseases (parasitic peristalsis stimulates nerves, causes nerve reflex and grinds teeth), and expel worms under the guidance of doctors. Stubborn cases should be made into mandibular pads and put on before going to bed. Can prevent teeth from being directly worn. This will not only make it possible not to grind your teeth at night, but also get a better rest, which is good for work, study and health.

Bruxism is mainly manifested in my inability to control my grinding and grinding activities. Due to long-term continuous grinding, teeth wear is serious, periodontal tissue is damaged, and all components of the entire oral and maxillofacial system are damaged. The principle of treatment is to block the cause and reduce the damage. The first is symptomatic and comprehensive treatment of the cause. Clinically, first relieve jaw muscle spasm, block occlusal interference, eliminate or alleviate bruxism, and then carry out other treatments.

(1) Psychotherapy: Bruxism patients do have the effect of psychological factors, which make the mandibular muscles overstretch. Such as emotional tension, anxiety, anxiety or fear of bruxism. It is very important to comfort and enlighten these patients and relieve their ideological concerns for establishing confidence in treating diseases.

(2) Muscle relaxation therapy: excessive mandibular muscle tension is one of the causes of bruxism, and relieving excessive muscle tension during treatment is a necessary means to control bruxism. The commonly used methods are: using muscle relaxation instrument; Physical therapy, training the physiological function of masticatory muscles; Massage; Audio-visual suggestion and other methods.

(3) Application of bite plate.

(4) occlusal adjustment therapy: through adjusting and polishing a small amount of tooth tissue, the occlusal interference and early occlusal contact can be relieved, and the occlusal balance relationship of natural teeth can be established, so as to achieve the physiological balance among occlusion, masticatory muscles and temporomandibular joint and eliminate bruxism.

(5) Bruxism can also be treated symptomatically, and patients with malocclusion should be treated with orthodontics first. Some need to repair or treat periodontal tissue diseases.

Sleep molars, or molars, refer to the rhythmic contraction of masticatory muscles during sleep, and the upper and lower teeth tightly bite and slide, making a loud creaking sound. People won't wake up at this time. Even if they wake up, they don't know their teeth. Only roommates know.

Sleep molars generally occur in the shallow sleep stage (the second stage). Molar won't appear in dreams. At this time, only physical activity and heart rate increase. When I wake up the next day, I often feel cheek pain and headache, sometimes accompanied by periodontal tissue damage.

The incidence of sleeping molars in children and adolescents is high, which can reach about 15% according to medical statistics. With the growth of age and the gradual improvement of brain function, its attack rate will decrease year by year and heal itself when it grows up.

However, if a child suffers from ascariasis, some neurological symptoms will appear after the toxins and metabolites produced by ascariasis are absorbed by the human body, and grinding teeth is one of the symptoms. Therefore, if a child is found to have ascariasis, it is necessary to use drugs to repel intestinal worms, such as Clivia, Melia azedarach root bark, betel nut and other traditional Chinese medicines have the best insecticidal effect.

Traditional Chinese medicine believes that the brain is the house of Yuan God, and all spirits are integrated. Sleep is related to the movement of yin and yang. If you sleep well, but you have abnormal behavior (including grinding your teeth) during sleep, it is all related to the role of the brain. It is advisable to tonify the kidney and marrow, strengthen the brain and soothe the nerves.
